Are you a compassionate and experienced Social Worker looking to make a positive impact on the lives of adults in need? 

As a Qualified Social Worker on our Adult Community Team, you'll play a pivotal role in supporting and enhancing the well-being of adults within our community. Key responsibilities include:

• Conducting comprehensive assessments to identify the needs of adults and formulate appropriate care plans.
• Collaborating with healthcare professionals, families, and external agencies to ensure holistic support.
• Advocating for individuals to empower and promote their independence and quality of life.
