Are you an experienced Social Worker looking for your next challenge in Adult Services? Wakefield Council is seeking a dedicated Assessment and Triaging Social Worker to join their Adults Team, supporting individuals to access the right care at the right time.

About the Role:

• Conducting strength-based assessments to determine eligibility and care needs.
• Working within a fast-paced triage environment, ensuring timely responses to referrals.
• Collaborating with multi-disciplinary teams, including health professionals and community services.
• Making informed decisions under the Care Act 2014, Mental Capacity Act, and other relevant legislation.
• Ensuring a person-centred approach that promotes independence and well-being.
   

About You:

Qualified Social Worker with Social Work England (SWE) registration.
Experience in adult assessments, triaging, or hospital discharge.
Strong knowledge of Care Act 2014, Mental Capacity Act, and Safeguarding Adults procedures.
Ability to work under pressure and make sound decisions quickly.
A proactive and solution-focused approach to social work.
